This will allow for access to additional information intended for first responders and those involved in our pre-hospital emergency care system. Please use your official agency email address in the email field.

Password / Logon Issues

The "Forgot Password" function only works when it matches your user name exactly. If you cannot remember your user name, please send an email to webmaster@polkfl.com . We will manually send you an email with your user name and new password. Please do not re-register if you already have an account with us. These emails, as well as new account activations are a manual process, so there may be some delay in processing them.
